The aim of the treatment of premature ejaculation is to facilitate learning voluntary control over ejaculation by fostering the patient's awareness of, and increasing his tolerance for, the pleasurable genital sensations that accompany the intense sexual excitement which precedes orgasm. Any method which encourages the man's concentration on pre-orgastic erotic sensations, including the "squeeze" and the "stop start" methods, can be used to implement this goal. The following variation of the stop start method is employed by our group: The patient is advised to concentrate on his erotic sensations only and not to attempt to "hold back."

1) The partner stimulates the patient's penis manually, until he feels that he is near climax. He then asks her to stop. A few seconds later, when he feels the acute sensations diminish, he asks her to start again. He climaxes on the fourth period of stimulation.

2) The stop start stimulation of the penis manually by the partner is repeated, this time using a lubricant. We use petroleum jelly. This procedure simulates the sensations produced by the vaginal environment.

3) Stop start stimulation is conducted intra vaginally, in the female superior position. The woman straddles her partner with his erect penis inside her vagina. His hands are on her hips to guide her motions. She moves up and down until he feels he is near climax and motions her to stop. Then he signals her to start again. He climaxes on the third period of stimulation. At first he does not thrust during this exercise. After he attains control he proceeds to thrusting.

4) Stop start intra vaginally is conducted in the side to side position.

5) Stop start intra vaginally in the male superior position.

6) Stop slow intra vaginally. After the patient has learned control by stopping at a high plateau of excitement, if necessary he can improve his control by slowing rather than coming to a complete stop at high excitement levels. This "stop slow" sig may be employed when the man is having difficulty integrating his new ejaculatory control into his behavior.
