The Steps to Shared Orgasm

1) Self stimulation to orgasm in presence of partner. This should occur with a gradual increase in intimacy. First the partner turns his back while the patient stimulates herself. Then he can hold her while she stimulates herself. One partner can stimulate herself to orgasm after the other has climaxed, but the patient should not attempt to reach orgasm during intercourse at this stage of treatment.

2) Manual genital stimulation by the partner to orgasm, while the patient uses his/her customary fantasy for distraction. (This is preceded by sensate focus I and II as needed)

Coital Anorgasmia

Treatment for this complaint essentially consists of progressive in vivo desensitization to orgastic release during vaginal containment. The treatment procedures for males and females are slightly different at this point.

Retarded Ejaculation

1) Orgasm is reached by manual self stimulation of genitals in the presence of the partner. This is done progressively nearer to the partner's vagina, until orgasm can comfortably be achieved near the mouth of the vagina.

2) Orgasm is achieved by manual stimulation by the partner.

3) The partner stimulates the man's penis manually to a point close to orgasm. Then the penis is inserted into the vagina at the moment of orgasm.

4) The procedure is repeated but the penis is inserted into the vagina progressively earlier until the need for manual stimulation diminishes.

5) Concomitant manual and vaginal stimulation of the penis. The female stimulates the base of her partner's penis manually as he thrusts in and out of the vagina. This maneuver supplies additional stimulation. It is sometimes used to supplement the progressive desensitization, described in steps 1 through 4.

Partial Ejaculatory Retardation

The immediate antecedents of this syndrome are similar if not identical to the immediate causes of general retardation of the entire male ejaculatory reflex, and treatment follows the same tactics: relaxation and stimulation concomitant with distraction by fantasy.

Female Coital Anorgasmia

The aim of the treatment of female coital anorgasmia is to diminish any anxieties, if these exist, that are provoked by penetration because this will lower the woman's orgastic threshold. In addition, the patient is trained to maximize the relatively low level of clitoral stimulation provided by penetration, and to accustom herself to have orgasms in this manner. Most woman can learn to have orgasm on coitus with concomitant clitoral stimulation with these techniques. But only a relatively few are successful in achieving coital orgasm without "clitoral assistance."

The Method Employed for the Treatment of Coital Anorgasmia

1) Self stimulation with pelvic thrusting. Under usual circumstances women reach orgasm during coitus by actively thrusting the pelvis against the partner's pubic bone and not, as during masturbatory, manual or oral stimulation, by passive reception of stimulation. Therefore, coitally anorgastic women are first taught to stimulate themselves to orgasm by thrusting their pelvis down against their stationary hand. This pattern of orgastic release improves the probability of learning climax during coitus.

2) The bridge maneuver. This consists of concomitant clitoral stimulation and vaginal intromission. While the 'penis is inserted, the woman or her partner stimulates her clitoris until she reaches orgasm. This is most comfortable in the side to side or rear entry positions.. Intromission is delayed until the woman is highly aroused by foreplay.

3) Progressively earlier cessation of clitoral stimulation while the penis is inserted in order to climax without "clitoral assistance." Many apparently normal women do not learn to climax during intercourse without clitoral assistance, at least by these methods. Such couples are counseled to adapt their love making to the woman's pattern of climax, without considering this "settling for second best."
