Psychogenic ejaculatory pain and post ejaculatory pain and the female dyspareunia caused by mild vaginismus are always caused by an involuntary, intense, painful spasm of certain genital muscles.

A variety of deeper or more inaccessible causes may be associated with this syndrome. In depth studies of female patients have revealed a variety of sources: guilt about intercourse and erotic pleasure, fear of penetration, and anger at the partner. In males the underlying causes range from mild masturbatory guilt to profound and severe sexual conflict. Sometimes no underlying cause can be detected.

Premature Ejaculation

In most cases, premature ejaculation is associated with the man's failure to perceive the erotic sensation of the late excitement stage which occurs prior to orgasm. Premature ejaculators will often report that they experience an atypically steep curve of excitement. Instead of the gradual rise of excitement, which many normal men experience, they go from low levels of excitement directly and steeply to the intense sensations of excitement which are premonitory to orgasm. Early mild excitement is well tolerated by these men, but high levels of excitement seem to evoke anxiety and its defense which in this case is repression, suppression or distraction from the clear perception of the erotic sensations. This defense mechanism or immediate antecedent produces the symptom of inadequate ejaculatoy control, for the perceptual defense against the sensations of erotic pleasure interferes with the normal process of learning ejaculatory control. It may be speculated that in order to acquire control over this or for that matter any other reflex, one must be able to experience and register the sensations that occur premonitory to the discharge of that reflex. Without the perception of orgasm together with high excitement, the process of sensory integration upon which learning is based is lost. The same is true of learning control of urination. The child must be able to feel and register the sensations of a full bladder if he is to learn control.

While the immediate cause of premature ejaculation is specific, the original or remote causes responsible for the anxiety evoked by intense sexual excitement and for the erection of perceptual defenses against his are highly variable in content. In my clinical experience, a wide variety of remote causes may be associated with inadequate ejaculaory control. Most of these patients get anxious when sexual pleasure s prolonged and intense, but the reason for this varies. Some, as Masters and Johnson pointed out, have trained themselves to climax quickly because of the pressured conditions of their early sexual experiences. Some have performance anxiety, fearing they will come to quickly or lose their erection, while some are angry at the partner or ill at ease with her. Many have anxiety related to all pleasure they tend to eat as quickly as they climax and do not relish their food my more than their sensuous feelings. Others seem to suffer from the remnants of unresolved oedipal and preoedipal and sibling rivalry problems, and infantile transferences which cause them to feel anxious then pleasure is too great and lasts too long. But in the majority of these patients no matter what the source or content of the anxiety — the quality of this sexual anxiety is mild or moderate and easily extinguished.
